Spicy Food Waiver

In consideration of Producer agreeing to consider including me as a potential participant in one or more episode(s) of "Hot Dates" ("Program"),  I hereby accept any risks involved in eating the food provided, accept all liability, and will hold harmless this business or the restaurant for any injuries to my person.  This release of liability includes, but is not limited to, damages or injuries incurred due to the effects of spicy food or its overconsumption.

I understand that the spicy food provided by the program may contain high levels of spice, peppers, and/or high levels of Scoville heat units. I further acknowledge that ingesting the spicy food provided involves risks that cannot by fully avoided due to the nature of the food and its ingredients. These risks include, but are not limited to:

• Burns to the mouth or skin.

• Burning or stinging in the eyes or other body parts if I touch my person immediately after consuming the food.

• Painful evacuation of bowels.

• Temporary or ongoing stomach upset or pain.

• Gas/flatulence.

• Elevated heart rate, blood pressure, sweating, or other physical response to the spicy food.

• Experiencing anxiety, stress, or other emotional and physical responses to the activity of eating the spicy food.

 

By signing below, I agree to waive liability for damages or injuries. Specifically, I will be liable for any and all medical costs, attorney fees, and other costs. I also assert that I am in sound physical condition for consuming the spicy food provided, and am free of heart ailments, high blood pressure, vascular issues, digestive issues, allergies, or other issues that could be exacerbated by eating spicy food provided. I am not under the influence of alcohol or drugs that could impair my judgment or my ability to safely eat the food.
